Tracking your COAP/COSSAP Data

This form is to be filled out once every quarter and requires information from social media accounts (Facebook, Instagram, Twitter, Etc.). Anti-Stigma, Peer-Recovery, Law Enforcement Agency & Recovery Services Initiative

To be used during a training event you host. To be completed  after a training event you participated in or hosted.

This form is to track a single event in which your agency either hosted or participated in a community event where you shared information about your program. If you participated in more than one event per quarter, please complete this form separately for each event.

This form will keep track of your overall program goals. Please complete this form separately for each project related goals.

This form is to be utilized any time technical assistance is utilized. This includes meetings with the DFA-IGS, State Drug Director’s Office, and Regional Prevention Representatives (RPRs).

This form is to summarize the accomplishments of a Prescription Drug Take Back Day and is to be filled out after the event has occurred.
